Good Afternoon, My name is Kelly Kerr and this is my Remote Collaboration and
Evidence Based Care video for NURS-FPX4030: Making Evidence-Based Decisions.
For this video I will be addressing the following:
 Propose and evidence-based care plan to improve the safety and outcomes
for a patient based on the Villa Health Remote Collaboration on EvidenceCare scenario.
 Explain the ways in which an evidence-based practice model was used to
develop the care plan.
 Reflect on which evidence was most relevant and useful when making
decisions regarding the care plan.
 Identify benefits and strategies to mitigate the challenges of interdisciplinary
collaboration to plan care within the context of a remote team.
 I will be making mention of authors from supporting literature.
First I will review the Villa Health scenario. In this scenario Caitlyn, a two-year old
girl had been readmitted to the hospital for a second time in six months for
pneumonia. The nurse Virginia Anderson is caring for Caitlyn and she is reporting
to Dr. Copeland of her findings. Caitlyn presents with decreased breath sounds in the
right bases and rhonchi scattered in the upper lobes. She also has shallow breaths
with respirations at 32 and a temp of 101. She has also been observed with a weight
of 20.7 pounds with a decrease of subcutaneous tissue on her extremities. Her
mother stated when she kisses her she tastes salty. A sweat chloride test resulted
with 65meq/L. At this point Dr. Copeland suspects cystic fibrosis and now begins to
work with an interdisciplinary team to help with Caitlyn’s new diagnosis. Rebecca
Hugo is the hospital respiratory therapist that will now be apart of the team. The
team had identified the barriers that Caitlyn’s family will face when caring for her
and this new diagnosis. The team also was able to brainstorm some solutions that
would provide the best patient-centered care for Caitlyn. The following is the
suspected barriers they will face:
 Caitlyn’s family living in a town that has limited access to healthcare.
 Carrying out the respiratory care Caitlyn will need daily to help improve her
respiratory function and prevent any respiratory distress.
 Social issues that Caitlyn’s parents face such as financial support, time
available to stay persistent in the respiratory treatments she needs, but also
the support her parents will need dealing with this new chronic illness
diagnosed for their child.
 Preventing complications a patient with cystic fibrosis may encounter such as
Distal intestinal obstruction syndrome also referred to as DIOS.
With all the different barriers that were identified it was important for the initial
team to address each barrier and find a solution, the best way to ensure everything
was covered was to address each issue individually and then make Caitlyn’s patientcentered care plan. Not only was it just Dr. Copeland, Virgina and Rebecca working
on it they now had to include Caitlyn’s pediatrician Dr. Benjamin and also social
work to check insurance coverage and availability for everything the family would
need for her treatments.
I will now discuss the evidence based care plan that can improve Caitlyn’s
health
According to Horsley, Cunningham and Innes citing from 2015, Cystic Fibrosis is not
only just a respiratory disease but can also affect multiple systems within the body.
Its classic hallmark sign is progressive and irreversible bronchiectasis and
respiratory failure. This disease can also affect the gastrointestinal function,
nutrition, endocrine and metabolic systems. The success of Caitlyn living a normal
life with this condition will depend on the evidence based care plan. Important
topics for her care plan should include preventing complications, identifying
knowledge deficits also addressing any social issues that can impact her care but
most importantly maintaining oxygenation.
The four major barriers that were mentioned earlier I will now address with
evidence based practice along with utilizing the American Nurses Association
Standards of Practice also known as ANA from 2015. The focus of standard of
practice for nurses includes Assessment, diagnosis, outcome identification, planning
and implementation. At this point we have already done the assessment, diagnosis
and outcome identification, now we are addressing the planning phase.
In regards to addressing the barrier of planning for Caitlyn’s family living in a remote
area with limited healthcare access:
 With Caitlyn’s family living in a remote area that alone is huge barrier that
could limit their ability to obtain care for Caitlyn along with having the
availability of resources. Telemedicine is something they can utilize for
Caitlyn’s care.
 According to Barrett, citing from 2016, telemedicine is defined as using video
in real-time for consultations between providers and patients.
 According to Wiesen citing from 2019, telemedicine is now being utilized for
chronic disease management. It
